The rate of diagnosis of Attention Deficit Hyperactivity Disorder among children has nearly doubled in the past two decades. Rates of A.D.H.D. diagnoses also vary[각주:1] considerably[각주:2] across states, with nearly three times as many children getting the diagnosis in Kentucky (where one in five children are said to have the condition) as in Nevada. More than 5 percentof all children in the United States now take an A.D.H.D. medication. These facts raise the question of whether the disease is being overdiagnosed.

 과잉행동 주의력 결핍장애를 진단 받는 아동의 수가 지난 20년간 2배로 늘었다. A.D.H.D.를 진단하는 비율 또한 주에 따라 현저히 다르다. 켄터키 주에서는 네바다 주에 비하여 약 3배 이상의 아이들이 A.D.H.D.를 진단 받는다. (캔터키 주에서는 5명중 1명의 아이들이 질병을 가지고  있다고 전해진다.) 미국 전역 에서는 5%이상의 아이들이 ADHD 약물을 복용하고 있다. 그러한 사실은 그 질병이 과잉 진단 되는 것은 아닌가에 대한 의문을 제기했다. 

Diagnosing A.D.H.D. is difficult. Unlike other childhood diseases — such as asthma[각주:3], obesity[각주:4] and diabetes[각주:5] — the diagnosis of A.D.H.D. is inherently[각주:6] subjective[각주:7] and depends on the assessment[각주:8] of parents, school personnel[각주:9] and health care providers. For a child who is easily distracted, an assessment of normal, inattentive[각주:10] behavior by one health care provider could be a formal diagnosis of A.D.H.D. by another.

ADHD를 진단히는 것은 어렵다. 천식이나 과체중, 당뇨병과 같은 다른 아동기 질병과는 다르게 ADHD 진단은 본질적으로 주관적이고 부모, 학교 직원,건강 보험 제공자의 평가에 의존하고 있다. 쉽게 집중력이 흐트러지는 아이나 부주의한 행동을 보이는 아이들은 증상이 보통이라고 평가 될 수도있지만, 어떤 사람들에겐 ADHD 평가 될 수도 있다. 

It turns out that although diagnosing A.D.H.D. requires a subjective interpretation[각주:11] of facts by physician[각주:12]s, the month in which a child is born can be a strong, objective[각주:13] predictor[각주:14].

비록 ADHD를 진단하는 것이 의사의 주관적인 사실 해석을 요구함에도 불구하고 이것은 아이들이 태어난 달이 강력하고 객관적인 예측 요소가 될 수 있다는 사실을 드러냈다. 

Most states have arbitrary[각주:15] cutoffs for kindergarten entry, such that children who do not reach a given age by a certain date are required to wait a year. In 18 states, children who will turn 5 before Sept. 1 can enter kindergarten in the year that they turn 5; children who will turn 5 after Sept. 1 must wait until the next year. So in states with Sept. 1 cutoffs, in any given class, August-born children will usually be the youngest and September-born children the oldest.


 대부분의 주는 유치원 정원을 임의적으로 마감한다, 예를 들어 특정한 날짜에 도달하지 못한 아이들은 일년을 더 기다리는 것이 요구된다. 18개의 주에서는 9월 1일 전에 5살이 되는 아이들은 그들이 5살이 되는 해에 유치원에 입학 할 수 있다. 9월 이후에 5살이 되는 아이들은 이듬해까지 기다려야 한다. 그러니까  9월1일로 마감하는 주에서는 ,어떤 학급 가정하더라도 , 대게 8월 생 아이들은 가장 어린 아이들이고 9월 생 아이들은 가장 나이가 많은 아이들이다. 


These arbitrary entry-age cutoffs have important implications[각주:16] for the diagnosis of A.D.H.D. In a study published in The New England Journal of Medicine, we found that among several hundred thousand[각주:17] children who were born between 2007 and 2009 and followed until 2016, rates of A.D.H.D. diagnosis and treatment were 34 percent higher among children born in August than among children born in September in states with a Sept. 1 school entry-age cutoff. No such difference was found among children in states with different cutoff dates. The effects were largest among boys.

 이러한 임의적인 입학 나이 마감은 ADHD 진단에 중요한 영향을 끼친다. 약물 뉴 잉글랜드 저녈에서 출간 된 새로운 연구에서, 우리는 2007년부터 2009년 까지 태어난 십 만명의 아이들을 찾아 2016년까지 ADHD 진단과 치료 비율을  추적하였다. 그 결과 9월 1이에 정원 마감을 하는 주에서 8월 생 아이들이 9월 생 아이들보다 34 퍼센트 높은 수치를 나타냈다.  다른 마감 날짜를 가진 주에서는 차이점이 드러나지 않았다. 이것은 남아들 사이에서 더욱 크게 적용 되었다.  

We believe these findings reveal just how subjective the diagnosis of A.D.H.D. can be. In any given class, inattentive behavior among younger, August-born children may be perceived[각주:18], in some instances[각주:19], to reflect[각주:20] symptoms of A.D.H.D., rather than the relative[각주:21] immaturity that is biologically determined and to be expected among children who are nearly one year younger than September-born classmates.

 우리는 이 결과가 ADHD의 진단이 얼마나 주관적일 수 있는가를 드러낸다고 믿는다. 어떠한 학급을 가정하더라도, 더 어린 아이들 사이의 부주의한 행동, 8월 생 아이들이 아마 인지될 것이다, 몇몇의 사례에서, ADHA 증상 으로 반영 된다. 다른 학생들 보다 상대적으로 발달이 덜 된 것으로 인지되기 보다 생물학적으로 결정되 어있다고 인지된다.  그리고 예상된다 9월에 태어난 학급 친구들 보다 거의 한 살 이 어린 아이들 사이에서

The stakes of additional, potentially inappropriate diagnoses are high, particularly[각주:22] when diagnoses are accompanied[각주:23] by medical treatment, which has side effects. In cases where A.D.H.D. is appropriately diagnosed, we know that behavioral and medical treatments can improve concentration[각주:24], school performance and other outcomes. And in these instances[각주:25], the harms of medical treatments are, on average, outweighed by the benefits. But when the disease is improperly[각주:26] diagnosed, the clinical harms and dollar costs of treatment may not be met with commensurate benefits.

Unlike other diseases such as asthma and diabetes, whose diagnosis is more objective and is not based on peer-to-peer comparisons[각주:27], the diagnosis of A.D.H.D. appears heavily influenced by how children behave in school relative to peers and how those differences in behavior are interpreted[각주:28] by school personnel, parents and ultimately[각주:29], physicians. Indeed, some evidence suggests that teachers and other school personnel are more likely than physicians or parents to first suggest that a child may have A.D.H.D.

  객관적이고 다른 사용자간의 비교에 기반을 한 것이 아닌  천식이나 당뇨병같은 다른 질병과는 다르게, ADHD의 진단은 아이들이 학교에서 친구들과 어떻게 지내는가 그리고 그러한 행동간의 차이점들이 학교 직원들과 부모들 결과적으로는 의사들에게 어떻게 이해되는가에 따라 크게 영향을 받는다. 실제로 어떤 증거들들은 선생님들이나 다른 학교의 직원들이 의사나 부모들에 비해 처음으로 아이가 ADHD일수도 있다는 의견을 제시하는 것으로 드러났다.

Our findings aren’t new, but they suggest a continuing problem. Several older studies, both within and outside the United States, analyze rates of A.D.H.D. diagnosis among children born just before versus just after school entry-age cutoffs, similar in design to our study. Nearly all of these studies suggest that younger children within a grade are more likely to be diagnosed with A.D.H.D. than older children in the same grade. One studyfound that the relative age of a child in a class strongly affects teachers’ assessments of whether a child demonstrates[각주:30] A.D.H.D. symptoms but does not affect parents’ assessments, which suggests that many diagnoses may stem[각주:31] from teachers’ perceptions of students that are based on a child’s age relative to peers.

Our study, which uses recent data, tells us that the problem still exists and that it’s not small. Despite[각주:32] growing awareness that A.D.H.D. may be overdiagnosed and the fact that the medications used to treat it have serious side effects, something as arbitrary as the month a child is born still has a meaningful impact on the likelihood[각주:33] that the child is determined to have the condition.

At a minimum, physicians who frequently diagnose A.D.H.D. in children should be aware of these findings. A simple mental “adjustment[각주:34]” for whether a child is born in August may be sufficient to help physicians reduce overdiagnosis.

School personnel and parents should also be aware of how simple cognitive [각주:35]biases[각주:36] can creep[각주:37] into how important clinical decisions are made. Both our and previous findings suggest that parents of children who are young for their grade could reasonably question whether the initiation[각주:38] of medical treatment for A.D.H.D. should be delayed.

최소한 아이들에게 빈번하게 ADHD를 진단하는 의사들은 이러한 결과들에 대해 인지하고 있어야 한다. 아이들이 8월에 태어 났는지 아닌지가 의사들의 과잉진단을 줄일 수 있는데에 큰 도움이 된다는 간단한 마음 가짐의 변화. 학교 직원들과 부모 또한 의식의 편향이 얼마나 쉽게 중요한 임상적 결정이 만들어지는데에 서서히 영향을 끼칠 수 있는지에 대하여 인지하여야 한다.  현재와 과거의 연구들 모두 본인의 학년보다 어린 자녀를 둔 부모에게는 약물 적 치료를 할지 말지에 대한 결정이 미루어 져야 하는가에 대한 합리적인 질문이 될 수 있다

In his 2008 book “Outliers,” Malcolm Gladwell describes the now well-known phenomenon[각주:39] that a disproportionate[각주:40] number of Canadian professional hockey players have birth dates in the beginning of the calendar year. This is explained by the Jan. 1 age eligibility[각주:41] cutoff for hockey programs in Canada, which leads to the oldest hockey players within an age-based division[각주:42] exceeding[각주:43] the age of the youngest players by nearly a year, conferring[각주:44] them a performance advantage[각주:45]. A similar phenomenon is true for A.D.H.D., where a child’s age relative to peers confers a markedly [각주:46]different rate of diagnosis and treatment, but the stakes are higher.

 캐나다 프로 하키선수들의 생년월일의 불균형이 년도의 사작에 따라 시작하는 것에 대한 현상을 밝혀낸것으로 잘 알려진 말콤 글래드웰은 2008년 그의 저서' 아웃라인즈'에서 이렇게 묘사하였다. 그것은 매년1월로 나이 적격성이 마감되는 캐나다의 하키 프로그램이 나이 기반의 분단 안의 가장 나이가 많은 하키선수가 리드하는 일년 가까이 어린 가장 나이가 어린 선수에 비하여 대단한 수행 능력적 우위를 수여한다는 사실을 설명 하였다. 비슷한 현상도 같은 학급 아이들에 비교한 아이의 나이가 ADHD의 진단과 치료에 대하여 현저하게 다른 비율을 가진 ADHD 의 경우에 진실이다. 그러나 이러한 현상은 더욱 커지고 있다.
